Health Resource Directory connects people diagnosed with a health condition with information and resources. The information is developed from the clinical guidelines GPs use in South Western Sydney.
The information has been reviewed and endorsed by local community members and GPs.
For health conditions, you will find:
A brief description of the health condition or procedure
What you can expect your GP to do now or in the future to help you manage the health condition
What steps you can take to manage your health condition
When you should call an ambulance or go back to your GP
What health and support services are available in South Western Sydney
Some questions you may want to ask your GP to better understand your health condition
Links to more information about the health condition
For procedures, you will find:
A brief description of the procedure and what it is used for
What preparations you will need to complete before having the procedure
What happens during the procedure
What you can expect afterwards
What you will not find on Health Resource Directory

The information has been developed from the same clinical guidelines GPs use in South Western Sydney. It has been reviewed and endorsed by the SWSPHN Community Advisory Committee and local GPs.
Most Health Resource Directory content is available in multiple languages in both text and audio files.
