Telemonitoring is a way of providing health services to people with chronic health conditions to keep them out of hospital, by working with them in their home and their GP, using electronic health monitoring tools.

These tools allow people to perform a routine test and send the data to a healthcare professional in real-time. Some examples of these mobile medical devices are glucose metres for people with diabetes, and heart or blood pressure monitors for people receiving cardiac care.

The following video was recently filmed with a Wollondilly resident currently using the telemonitoring device. The video clearly shows how satisfied this resident is with the program.
